If at the time of your fall, you completed and incident report and have not received a copy of that report, I’d recommended that you ask the manager of the location where you fell for that incident report. You should also promptly write down everything you can remember about the reasons for your fall. If there were any witnesses, write their names, their addresses, and phone numbers if you have them as well. If possible, take photographs of the area where you fell. If you still have the shoes that you were wearing at the time you fell, stop wearing them and set them aside until your case is concluded.
