You’re wondering whether or not you have to do that or if it’s in your best interest to do that. The answer to the question turns on whether or not the request comes from your insurance company or the insurance company that insures the driver at fault in the accident. If it’s the insurance company for the driver at fault, the answer is no. You have no obligation to provide them with an authorization, and generally speaking, it’s not in your best interest to do so. If the request for the authorization comes from your insurance company, under the terms of your insurance policy you have an obligation to respond. You have a contractual duty to cooperate with your own insurance company, and included within that duty to cooperate is an obligation to make your medical records available.
